Care Service
Our Care Service delivers packages of specialist, outcome-focused support for adults, children and young people transitioning into adult services. Supporting people with a range of complex care and support needs including learning disabilities, autism, mental health, behaviours that challenge and chronic conditions, we enable each person to live in their own home and community
We are driven by our values, culture and person-centred approaches
Our specialist team uses their skills and expertise to deliver high-quality care and support for people with high complex care needs including behaviours that challenge, acquired brain injury, and mental health needs. Each package of care and support is tailored to meet the person’s individual needs and requirements, goals and aspirations.
We work closely with each person’s loved ones and extended network of support, to help them move towards greater independence and community engagement wherever possible. Personal care and home services form part of our service offering, with support provided 24/7 where needed.
We are regulated by the Care Quality Commission (CQC) and work hard to meet and exceed the fundamental standards of quality and safety that they set and rate us against.
Partnership working
We provide a multi-disciplinary approach to the care and support we deliver.
Working together with commissioning authorities and other key partners, we support each person to move towards taking greater control of their lives, supporting them to be able to choose how they would like to live their life.
